Before you make an Appointment

To make an appointment at San Mateo Physical Therapy Center you will need to gather the following information:

1. Personal Information including your call back phone number and your date of birth.

2. Your insurance information including Card ID number, group number, name and date of birth of subscriber if spouse or dependent; and the phone number of insurance.

3. The name and phone number of your referring physician.

4. Bring this information to our office along the referral script and an identification card such as your California Driver License or California ID.
